This company is a sales group for it's parent's company that manufactures custom metal ceiling panels. They are actually adding some manufacturing EEs but they don't know the proper W/C codes for those employees so I added them in on this RFP as 3066. This is a description of the work they perform (We manufacture metal ceilings using two machines - one bends metal into panels and one perforates metal. The employees control the machine via software and they insert metal sheets onto the machine rollers and remove the finished product from the other side of the machine. The employees also operate a fork lift, work with saws, air compressors, laser cutters, and other common warehouse tools.) You will need to work with underwriting to properly insure the correct code for these EEs. They are currently with a Professional Employer Organization that is providing them with Signa health care coverage, but that program only offers 2 plans and they would like to have more options. They contact did not want to expose their census to me so I've directed her to provide that documentation directly to you, the Professional Employer Organization rep. I do have a copy of their W/C dec attached to this RFP but I have not yet received their LRs. I have also requested the plan coverage summaries, recent invoice and renewal notice for that Signa coverage but I have not yet received those either so make sure you request those on your initial call. This RFP is actually for three separate business entities. The first is the private equity company, based in AZ, that owns the other two. Another is a high tech machine shop based in AR. The third is an electronics component manufacturer located in San Jose, CA. They currently utilize a Professional Employer Organization but have just figured out how much that Professional Employer Organization is costing them and they are now shopping for an alternative Professional Employer Organization. The employee count and payroll numbers listed on this RFP are a combination of all three entities, but a separate breakdown is included. This company is an English company that manufactures and installs a new type of building fire suppression systems. They currently have 4 US sales reps, and they originally contacted me just about procuring benefits. They have a US based accountant handling payroll and tax administration. I suggested they investigate utilizing a Professional Employer Organization for total HR admin and they agreed. Once UL testing is completed their plan is to hire as many as 40 additional employees for more sales, clerical, manufacturing and installation. This company fabricates and installs commercial HVAC duct work. It appears the company has been working under the HVAC W/Comp code instead of the duct work code, and is now being billed for the difference in premium by AM Trust. So if you quote AM Trust coverage, please let's discuss beforehand. I have received and attached to this RFP a copy of their current W/C dec, Unemployment rate doc, payroll report and a W/C claims history affidavit. I have replied back that the affidavit may work to start the quoting process but actual LRs will be needed. Please reiterate the need for actual LRs when you make contact.

Health participation is good with 50 of the 81 eligible EE's on the plan. They also make premium contributions across all dependent levels. The contact handles all the HR now, but has recently been taking on several other operational tasks. So, their main focus is to mitigate against future health increases, and shed most of the HR functions. They are not looking for a Jan 1st transition.

This company was recently pitched by a Professional Employer Organization rep and is now interested in obtaining at least one more competitive quote. This company is a manufacturer of folding cartons and air filter frames - they buy paperboard and convert it via printing, die cutting, and folding/gluing. Their growth is up significantly over the past 2 years but face capacity and hiring constraints. They believe their EEs are critical to their success and want to treat them right. They currently offer Anthem coverage which renews in May, but they are not looking for the Professional Employer Organization to quote their group medical at this point. If you feel you have particularly competitive group medical rates at this point, you may want to push to include medical rates in your proposal. An OH BWC release form has been provided to the contact to sign and return. They do have 2 EEs that live in KY but work at the OH facility. And they have one EE that is outside sales in MI and works out of his home.
