They did complete a Loss History affidavit and it is attached this RFP. This operation has been bought and sold many times over the last several years to as a way to help mitigate this new upheaval the new owner wants to use a Professional Employer Organization and he requires the new Professional Employer Organization to be able to supply large group United HC plan coverage options on par to their current coverage through Williams. Those plans summaries are attached to this RFP. A benefits census is currently being worked on and it will be provided to you as soon as it is received. They are looking for a Jan 1st start but they want to start their evaluation process ASAP. In addition to ND they will also have one EE in WY. They will also periodically use 1099 contractors and it would be helpful to them if their new Professional Employer Organization could assist in patrolling these contractors. The owner would like you to first introduce yourself via email and request a time to schedule your initial phone conversation. If you can, include your United plan desriptons you offer in ND on the email so that you can discuss during that initial call. Below are some additional notes from the contacts assistant.: *We are a petrochemical measurement and analytical laboratory based in Williston, ND. We currently have 19 employees; four of us work in the lab, two work from home (office staff), and thirteen employees are in the field taking samples, measuring oil flow, and calibrating gas meters. We expect to hire up to 6 more field technicians before the end of 2023. Right now, all of us are covered under the ND workers comp code 1320A. *Estimated employee count on January 1, 2024 is 25 with a total gross payroll of $3.3 million. Specializing in decks, porches, patios, pergolas, sunrooms, patios, walkways, retaining walls, outdoor water features etc. They have a couple Professional Employer Organization quotes already, but feel those companies are not truly capable of handling their type of business. Their primary interest is to get a better benefits plan in place. Their current benefits plan renews on Dec 1st, but your contact will consider another alternative (COBRA)to extend to Jan 1st 2018 if your case is compelling enough. This is a woman owned, fire and water restoration company that's been in business for 23 years. They are currently utilizing a Professional Employer Organization that they have typically had a good working relationship with. However, they have had a claims issue that they feel that Professional Employer Organization has not done a good job in advocating their interests. In addition, the client does government work on occasion, and their current Professional Employer Organization doesn't handle certified payroll processing well. So now they are looking into other Professional Employer Organization options that may be able to provide them with better administration. At this point, they would most likely be looking forward to a Jan 1st conversion. They offer full medical and dental. The company pays 60% of the medial. They use Benefit Pro and like them so far. Offered is Sharp Health Care through the company plan – they also can choose Kaiser or a couple of PPO’s. They just completed open enrollment last week. The attached documentation includes LRs, ACORD app, recent payroll report and a spreadsheet with a bit more accurate payroll breakdown by code.

This will be a new entity that is buying some of the existing well assets of another large operator. At this point, the company will be domiciled in LA where most of the clerical EEs will be located and the wells are located in TX. However, they are still evaluating organizational structures and may make the TX operation a subsidiary of the LA entity. If there are any implications on how or where these companies are domiciled, please advise the contact on those issues. In ten days the current owner will address the EEs of the pending change in ownership and the next day this new entity will address the same EEs, introducing themselves and describing the compensation package. It is for this meeting that they will need to have that package nailed down. It has been explained that the TX field EEs will only be operating currently producing wells. All drilling, water hauling, pipeline work and roustabout will be performed by outside contractors. The owners of the new entity perused a different group last spring which was eventually declined, but the attached PDF describes the benefit compensation package from that offer and they would want to mimic here. Please make sure to review it prior to initial contact. No census is available at this time. The current owner will not release any info until a deal is formalized. These guys are looking for a true partner that can help facilitate a smooth transition.

They initially engaged Insperity to provide all their HR admin and group medical, but after 2 weeks in, Insperity invoiced them for medical at a rate greatly over what was quoted. It also appears that Insperity was carving out their W/C as the company has a TX Mutual W/C policy. They now must find a new Professional Employer Organization and group medical to put in place by May 1st. Some supporting underwriting is attached to this RFP. The company was established in October 2015 as a privately held company providing production treating chemicals for oil and gas producers. We were established to assist producers in the Sonora-Ozona area in meeting their production challenges by generating revenue through innovative applications of specialty chemicals. We are a production chemical company which provides superior local service, specialized chemicals and innovated applications supported by 3 experienced personnel. This company is a full service environmental drilling & excavation contractor with vertical and horizontal drill rigs, excavators, bulldozers, etc. They drill monitor wells, water wells, and horizontal environmental wells. They also excavate contaminated soil and backfill with clean soil. They had one EE that made W/C claims each of the last three years, and it has resulted in 1.31 W/C EMR. That employee is no longer with the company. However, the company needs to show a lower EMR before it starts to lose contracts for them. They want to see how a Professional Employer Organization can help them with benefits, and risk management.

They don't actually perform any of the work, all work is subbed out. The 5606 EEs are the project managers and the 8720 EEs are estimators. The 9014 EE was just promoted to 5606, so they no longer have any 9014. They are looking for a Professional Employer Organization with a bit more technology that allows a more automated way to submit payroll and integrate with their accounting systems.
